A MUM-of-two wrote a heartbreaking final post before passing away from cancer.
Kate Rackham, who was diagnosed with incurable secondary breast cancer at the age of 39, tweeted “If you’re reading this, I’ve died” shortly after loosing her battle.

The mum-of-two, from Manchester, wrote the tweet and held it in drafts on her account named ‘Teacher with Cancer’ before sadly passing away.
In the emotional tweet posted on Thursday, she said: “If you’re reading this, it means I have died.
“But do not cry for me. I have lived my life on my own terms, the way I have wanted to.
“I joined X because I needed an outlet, what I got was so much more.
“You made me feel validated in my feelings and much less alone. Thank you.”
Kate would share updates and feelings during her cancer journey with all 14,000 followers she had amassed.
Kate is survived by husband Mark and their two daughters Ruby and Nancy.
