Childcare experts have expressed diverse opinions on the safety of breastfeeding babies while lying down.
While some believe that it can increase the risk of choking or suffocation, others argue that it is a natural breastfeeding position and beneficial practice.
The World Health Organisation advises that babies be breastfed within the first hour of delivery and should be exclusively breastfed for the first six months of life.
According to WHO, infants should be breastfed on demand as often as possible.
The United Nations International Children’s Fund listed cradle hold, cross-cradle hold, football hold, reclining position, and side-lying position as breastfeeding positions.
However, some experts have raised concerns over breastfeeding while lying down, warning that it can increase the risk of choking or ear infections in infants, decrease milk supply and cause discomfort for mothers.
Speaking with PUNCH Healthwise, the physicians insisted that breastfeeding in a lying position is a dangerous practice that can increase the risk of Sudden Infant Death Syndrome and other hazards.
A paediatrician at Federal Medical Centre, Abeokuta, Dr Kareem Salau, cautioned that if a baby is breastfed while lying flat on their back, their airway may become clogged, which might lower oxygen levels and raise the risk of sudden infant death.
He said, “When a baby is in a lying position during breastfeeding, the risk of accidental suffocation is increased, especially if the mother falls asleep.
“Other concerns include potential neck and back injuries for the baby, as well as an increased risk of ear infections and reflux.
“It is believed that the risks are simply not worth it, especially given that there are safer alternatives for breastfeeding, such as the traditional cradle-hold.
“The combination of lying position and breastfeeding creates a perfect storm of risk factors for sudden infant deaths and suffocation, especially for younger infants who have not yet developed the ability to move their heads or roll over.”
He noted that the risks of lying positions for breastfeeding are even more pronounced when the mother is tired or sleep-deprived.
“When a mother is exhausted, it is more likely that she will fall asleep while breastfeeding, which can result in her rolling on the baby or the baby being smothered by her body or blankets,” he warned.
Also, a Professor of Paediatrics and Child Health at the Obafemi Awolowo University, Ile-Ife, Osun State, Professor John Okeniyi said there is a high risk of choking if the milk flows too quickly and the baby cannot manage the flow while breastfeeding in a lying position.
The Chief Medical Director at Obafemi Awolowo University Teaching Hospital affirmed that it could lead to choking, and ear Infections, especially when the milk flows into the eustachian tubes during side-lying breastfeeding.
He added, “While rare, there is a concern that breastfeeding in a lying position could lead to these serious conditions if not done properly.
“Other health concerns include a decrease in milk supply. Some mothers find that their breasts don’t drain fully or evenly in the side-lying position, which could lead to engorgement or a decrease in milk supply. Incorrect positioning may cause discomfort for the mother.”
However, the physician argued that breastfeeding in a lying position could also be beneficial depending on the circumstances surrounding it.
He explained that breastfeeding in a lying position may be recommended for mothers recovering from childbirth, especially caesarean sections, or for those who need to rest
He added that the side-lying position can be more comfortable and facilitate night-time feedings.
He also said that premature babies may benefit from the laid-back breastfeeding position, which can be easier for them to manage.
“Side-lying breastfeeding can be beneficial for babies with reflux by promoting better digestion and reducing symptoms,” he added.
When asked about the alternative breastfeeding positions considered safer, the child health physician said, “Cradle hold is the most common position, offering control and comfort.
“Football holds breastfeeding position is useful for mothers who have had a cesarean delivery, while cross-cradle hold offers good control of the baby’s head for a deep latch.
“Healthcare providers should be aware of cultural practices and provide advice on safe breastfeeding positions while respecting these traditions. Education on the benefits and risks of different positions can help mothers make informed decisions.”
On her part, a paediatrician at the Olabisi Onabanjo University Teaching Hospital, Sagamu, Ogun State, Dr Sarah Ademola said lying positions can be safe and beneficial for both mother and baby, promoting bonding, relaxation, and breastfeeding success.
She stated that the benefits of breastfeeding in a lying position can outweigh the risks, especially when proper precautions are taken.
“Many mothers find that breastfeeding in a lying position is more comfortable and less stressful, leading to better bonding and increased breastfeeding success.
“It is so much easier to nurse my baby in a lying position at night. I know there are risks, but for me, the benefits have been worth it.
“For mothers who have had a difficult birth or are experiencing postpartum pain, breastfeeding in a lying position can help to ease pain and promote healing,” she explained.
