A toddler died after swallowing a plastic googly eye that was stuck in his body for 14 months.
Hospital scans failed to pick up the item that was lodged in two-year-old’s Kazarie Dwaah-Lyder’s body from February 2022.
Each time he went for an X-ray and fluoroscopy it came back negative and he was sent home by doctors, St Pancras Coroner’s Court heard.
The toddler from North Woolwich, London, died at Great Ormond Street Hospital on April 27 2023 because of bleeding from a hole in his oesophagus.
His medical cause of death was given as 1a upper gastrointestinal bleed and haemorrhagic shock, 1b oesophageal tear extending to the aorta, and 1c foreign body in oesophagus.
A post on Kazarie’s GoFundMe reads: ‘We are deeply saddened to inform you that our beloved child Kazarie Kwaku Dwaah-Lyder; aged 2, passed away on Thursday 27th April 2023 at 4:01am, at Great Ormand Street due to unforeseen circumstances and severe complications. (A hole found in his oesophagus).
‘Kazarie was an amazing, smart, funny and loving little boy who had an old soul, he touched the hearts of many, and we want to give him the farewell he deserves.
‘We understand that this is a difficult time for everyone, but we kindly ask for any amount that you can give, no matter how small, to help us honour his mum and older brother as well as extended family.’

Talking about the case, Coroner Mary Hassell told the court: ‘I was told that there is a lack of national guidance for such a situation.
‘I appreciate that there are multiple considerations in planning investigations, such as the risks associated with CT scanning and the risks associated with the administration of a general anaesthetic.
‘It seems that the matter would benefit from consideration at a national level.’

Ms Hassell’s report has been sent to the British Association of Paediatric Surgeons, the Royal College of Paediatrics and Child Health, and the Royal College of Radiologists.
The BAPS confirmed it had received the coroner’s report, and would be responding before the April 8 deadline given by the coroner.

An RCPCH spokesperson said: ‘This is an absolutely tragic case and our thoughts go out to Kazarie’s family.
‘I can confirm that RCPCH has received the coroner’s report and will be responding within the timeframe stated.’
Kazarie leaves behind an older brother as well as his parents.
